The Transformation Exchange was founded by Medhya Sahay to address a consistent problem seen across complex organisations: performance that depends on leadership intervention rather than structural strength. We provide structural performance consulting, operating model consulting, and organisational design consulting to improve how organisations actually run, not just how they are designed.

Our founding story

The problem wasn't effort. It was structural.

I founded The Transformation Exchange after repeatedly seeing the same pattern across organisations: strong strategies, capable people, and significant investment, yet inconsistent performance. Delivery depended on senior leaders stepping in to unblock issues, resolve escalations, and reconnect fragmented work.

Through years of working in complex transformation environments, including leadership advisory roles and large-scale programmes, I saw that many performance issues were misdiagnosed. Organisations focused on culture, capability, or technology, while the underlying structure, decision pathways, operating models, and cross-functional interfaces remained unchanged.

The Transformation Exchange was created to challenge that. It brings a clear, structured approach to diagnosing and improving organisational performance at its source. Instead of adding more activity, it focuses on strengthening the structural conditions that allow performance to hold without constant intervention.

It provides a focused, evidence-based assessment that delivers clarity quickly, without the disruption of a full transformation programme.

Medhya is a Business Transformation Director and former KPMG management consultant with over 25 years' experience working with complex organisations. She specialises in diagnosing structural performance issues and designing operating models that enable consistent, scalable delivery.

She developed the Structural Capability Index to support structural performance consulting and operating model consulting engagements.
